The Education Association of Moore (TEAM) is committed to ensuring every employee has a safe workplace. This confidential report helps TEAM:

  • Track incidents of workplace violence and assaults on employees of Moore Public Schools.
  • Identify patterns across the district of incidence of violence and assault.
  • Advocate for safer working conditions.
  • Connect members with support and resources available through Moore Public Schools (MPS).

Note: This survey does not replace required district reporting, workers compensation claims, or law enforcement reports. If you have been assaulted, please notify your administrator IMMEDIATELY.

This survey is only to be utilized to identify when assaults occur on a staff member. Please do not fill out this form if an assault occurred between students only. An assault or violence between students needs to be input into IC and handled by your administrator.
